Have already received the sacraments of Baptism, Reconciliation and Communion-A copy of the certificates are needed.
Have attained the age of fourteen (14) or older at the time of celebration of the Sacrament.
Families must have a record of regular attendance in A Family of Faith Formation program or a Catholic School for one full year is needed before a candidate can be enrolled for Confirmation preparation.
The second year the child must participate regularly in Family Faith Formation in addition to the special preparation sessions for Confirmation.
Family must be registered member of the parish

According to canon law, sponsors must have the following qualification (based on canon 893, which in turn refers to canon 874, a description of the qualifications for baptismal sponsors):
A sponsor must be interested in the spiritual well-being of the candidate.
A sponsor must be at least 16 years old. (We need to be aware that many sixteen-year old's do not have the maturity needed to do this.)
A sponsor must be fully initiated Catholic (one who has received the Sacraments of Initiation: Baptism, Confirmation, and Eucharist) who “leads a life of harmony with the faith and the role to be undertaken.”
A sponsor must not be bound by any Church penalty.
A sponsor must participate regularly in the sacramental life of the Church including weekly Sunday Masses.
A sponsor must not be the parent of the one who will receive the sacrament.
It is desirable that the one who undertook the role of godparent at baptism be sponsor for Confirmation and Eucharist.
Please note the following:
A sponsor should be someone with whom the Candidate can talk easily and who will model the faith for the Candidate.
A sponsor who is willing to share his/her own faith story with the candidate.
Sponsors are to be present at the Sacrament.
Sponsors and candidates should have regular meetings. Such as meeting once per month in a casual environment to discuss the candidate’s formation without much interruption.
If the Sponsor lives out of town, and cannot be available for regular meetings with the Candidate they can communicate by telephone, email or other forms of social media. If this still is not possible, the Candidate should also choose a Mentor who can be available for the regular meetings. The Mentor can serve as proxy if needed.
Sponsors must provide the following:
A baptismal certificate and a Confirmation certificate or a Baptismal certificate with all sacraments noted on the back.
IF the sponsor is married, a church marriage certificate will need to be submitted. (Certificates can be obtained by contacting the Sponsor’s Church of Baptism as all records in the Catholic Church are held by the Church of Baptism.)
If the sponsor is not a member of this parish, the sponsor must obtain a letter of good standing from his or her parish.
